IPT Community-Based Training Portfolio
SUBMISSION PORTAL
Please read the following submission information carefully
For certification, you must complete TWO portfolios (one portfolio for each of two different clients). Each portfolio must obtain a passing grade (average of 3.0 on a 1-5 scale) as rated by an IPT Institute accredited Consultant on the IPT Quality and Adherence Scale.
All materials must be submitted in English, although the audio or videotape may be in the client’s language if permission is obtained from the IPT consultant.
Please label your documents this way:
LAST NAME-FIRST NAME-SUPERVISOR LAST NAME-document-Case#1 (or 2 if appropriate)
For example, your Interpersonal Inventory would be labelled:
Smith-John-Belik-Inventory-Case#1
Once we receive your submission, we’ll review it for completeness and let you know if anything is missing. Complete portfolios will be sent to your consultant to review
Each portfolio must include the following.

A document recording the PHQ-9 Scores and GAD7 Scores of the client every 4 weeks, starting with session #1. You can upload individual PHQ-9 and GAD-7 questionnaires using this link repeatedly, or you can upload a summary of the scores. Smith-John-Belik-PHQ9 and GAD7-Case#1
An audiotape or videotape of one full session of IPT your treatment: Smith-John-Belik-Videotape-Case#1
Please upload your recorded session even if the audio is not in English.
